Abstract
Chronic liver disease (CLD) represents a spectrum of evolving hepatic dysfunction often leading to irreversible damage such as fibrosis and cirrhosis. Alcoholic liver disease (ALD) is the most prevalent and a major subclass of CLD globally. In ALD associated ascites, the ayurvedic approach of diagnosis involves identification of doshic imbalance and it can be correlated with Yakritodara. The management includes Shodhana, Shamana and Rasayana. This case report presents a case of chronic Liver Disease and Ascites. A 63-year-old male presented with progressive breathlessness, bilateral leg swelling causing discomfort and mobility limitations, abdominal heaviness, generalized weakness, and yellowish discoloration of the eyes. The patient reported a significant history of alcohol consumption for 40-years, and had a positive history of smoking. Pallor and icterus were evident, alongside digital clubbing. Abdominal inspection and palpation showed positive flank test, shifting dullness, positive fluid wave test, hepatomegaly and splenomegaly.
The patient underwent a structured Ayurvedic protocol targeting Jalodara complicating Yakrutodara, initially with Deepana Pachana, followed by Mridu Virechana and Rasayana. The pathya was followed with only buttermilk and rice gruel as the food all along the course of the intervention. After 10 days of intervention the patient was reassessed for the clinical signs of ascites and also the diagnostic investigations including ultrasound and laboratory investigations. The patient has exhibited significant clinical improvement and there was improved imaging study and normalised liver enzymes. This study establishes that Ayurvedic intervention (Vardamana Pipalli) as a clinically effective monotherapy.
